Von Hippel-Lindau Hastalığı Olan Bir Hastada Tek Aşamalı Bilateral Adrenalektomi (Kortikal Koruyucu) ve Pankreatektomi (Korpus Koruyucu)

Von Hippel – Lindau (VHL) hastalığı otozomal dominant aktarılan bir sendromdur ve birçok organı etkiler. Bu yazıda, eş zamanlı fonksiyon koruyucu adrenalektomi ve pankreatektomi yapılan bilateral feokromositoma ve çoklu pankreas nöroendokrin tümörü olan VHL hastasını sunmayı amaçladık. Hastaneye hipertansiyon yakınması ile başvuran 27 yaşındaki kadın hastanın bilgisayarlı abdominal tomografisinde sağ adrenal bezde 12x7 cm ve sol adrebal bezde 4x4 cm solid tümörü; aynı zamanda, pankreas başında iki ve kuyruk bölgesinde de üç adet solid kitlesi mevcuttu. 24 saatlik idrar metanefrin ve normetanefrin dışında tüm laboratuvar testleri normal idi. I-123 MIBG sintigrafisinde her iki adrenal bezde tutulum görüldü. Bu bulgular bilateral feokromositoma ve çoklu pankreatik nöroendokrin tümörü ile uyumluydu ve VHL tanısı genetik testlerle konfirme edildi. Alpha-1 inhibitor ile süpresyon sonrasında, sağ total, sol korteks koruyucu adrenalektomi, pankreas baş lezyonları için Whipple prosedürü ve dalak koruyucu pankreatektomi yapıldı, pankreas korpusu korundu. Bu olgu, VHL hastalığı olan hastalarda çoklu fonksiyon koruyucu prosedürlerin onkolojik prensiplerle güvenli bir şekilde uygulanabileceğini göstermektedir. (SETB-2023-04-059)

Single Stage Bilateral Adrenalectomy (Cortical-Sparing) and Pancreatectomy (Corpus-Sparing) in a Patient with Von Hippel-Lindau Disease

Von Hippel–Lindau (VHL) disease is an autosomal dominant syndrome and effects many organs. We aim to report an adult patient with VHL disease having bilateral adrenal pheochromocytoma and multiple neuroendocrine tumor of the pancreas who was successfully treated with simultaneous function preserving adrenalectomy and pancreatectomy. A 27-year-old woman was admitted to hospital with hypertension. The computed tomography of the abdomen was revealed solid tumor in both adrenal glands with the sizes of 12x7 cm on the right and 4x4 cm on the left. She also had two pancreatic solid masses in the head and three in tail with varying size. The laboratory tests are all within normal limits except elevated 24-hour urinary metanephrine and normetanephrine. I-123 MIBG scanning showed increased uptake in both adrenal glands. Fine needle aspiration biopsy of the tumor on head of pancreas via endoscopic ulrasonography showed neuroendocrine tumor. Those findings were compatible with bilateral pheochromocytoma and multiple pancreatic neuroendocrine tumors and genetic tests revealed the mutation which confirmed the diagnosis of VHL disease. After suppression with alpha-1 inhibitor, right total, left cortical-sparing adrenalectomy, Whipple procedure for the pancreatic head lesions and spleen-preserving distal pancreatectomy were performed, pancreatic corpus was preserved. This case showed that multiple function preserving procedures can be safely performed with oncological principles in patients with VHL disease.
