Limited Joint Motion in Adult Type Diabetes and Its Relation with HbA1 Levels

OBJECTIVE: In order to investigate the changes in the mobility of large and small joints with the age of diabetic patients, the duration of the disease and the quality of diabetic regulation, we undertook the following study.
STUDY DESIGN: In this study we examined 28 diabetic and 25 control patients. While the big joints of the right upper extremity were examined goniometrically, the finger joints were assessed either by putting the palms together or on top of a table. The quality of blood glucose control was assessed by the percentage of glycosylated haemoglohin (HbA1). Student's t-test and chi square test were used for the statistical analysis of the resul ıs.
RESULTS: Except external rotation of the shoulder arıd elbow extension, big joints of the right upper extremity were noı limited in diabetics when compared with controls. Flexion contracture of the finger joints were significantly (p<0.001) higher in the patient group. The limitation of extension at the finger joints increased both by the age of the patient in both groups and by the duration of the disease in the diabetic group. High HbA1 levels did not decrease the mobility of the big joints but in those patients who had poor control of their disease, finger flexion contractures were significantly (p<0.05) higher.
CONCLUSIONS: We conclude that limitation of the big joints of the right upper extremity due ıo adult type diabetes is possible hut is not affected by poor glucose regulation. On the other hand, finger joints are more severely affected by the presence of diabetes, by its regulation and the age of the patient.

Erişkin Tipi Diabette Eklem Hareket Kısıtlılığı ve HbA1 ilişkisi

AMAÇ: Bu çalışmada insilüne bağımlı olmayan erişkin tipi diabette büyük ve küçük eklemlerdeki hareket kısıtlılığı­nın hasta yaşı, hastalık yaşı ve diabet regülasyonundan ne yönde etkilendiğini araştırmak amaçlandı.
MATERYAL VE METOD: Çalışmaya alınan 28 diabetik ve 25 kontrol olgusunun sağ üst ekstremitelerdeki büyük eklemleri goniometre ile, her iki el parmak eklemlerindeki ekstansiyon kısıtlılığı ise avuç içleri birbirine veya masa üstüne değdirilerek değerlendirildi. Diabet regülasyonu HbA1 tayini ile izlendi. Sonuçların istatistiksel anlamlılığı student’t t-testi ve chi kare testi ile araştırıldı.
BULGULAR: Diabetiklerin omuz dış rotasyonu ve dirsek ekstansiyonu dışında, büyük eklemlerinde kontrollere göre anlamlı kısıtlılık saptanmadı. Parmak eklemlerindeki fleksiyon kontraktürü diaberiklerde p<0.001 düzeyinde anlamlıydı. Parmak eklemlerindeki kontraktür hem hasta hem de kontrol grubunda yaşla birlikte, diabetiklerde ayrıca hastalık süresiyle birlikte anlamlı artmaktaydı. Diabet regülasyonunun kütü veya iyi olması büyük eklemlerdeki hareket açıklığını etkilemezken, regülasyonu kötü olanların parmak eklemlerindeki fleksiyon kontraktürü iyi olanlara göre p<0.05 düzeyinde anlamlı yüksekti.
SONUÇ: Erişkin tipi diabet sağ üst ekstremitedeki büyük eklemlerde kısıtlılık yapmakla beraber bu, diabet regülasyonu ile ilgili bulunmamıştır. Parmak eklemlerinde ise diabetin varlığı süresinin uzunluğu kötü regüle edilmesi ve ayrıca hasta yaşı anlamlı derecede ekstansiyon kısıtlılığı­na neden olmaktadır.
