MRI Findings of Patients with Partial Epilepsy

OBJECTIVE: In this study we have investigated the role of magnetic resonance imaging (MRI) in determining the etiology in 63 patients with partial epilepsy.
MATERIALS AND METJIODS: The MRI findings of 63 patients with partial epilepsy followed by Epilepsy Outpatient Department of Şişli Etfal Hospital Neurology Clinic were investigated retrospectively. The type of seizure was detemıined by the informatiorı taken from the patients and their relatives who had seen the seizure. The electroecephalogram was recorded twice yearly in each patient. All patients were screened in T1, T2 and proton density.
RESULTS: Thirty-two patients were female and 31 were male. The seizure type was simpily partial in 6 patients, complex partial in 14 patients and secondarily generalised in 43. EEG was pathologic in 47 patients and normal in 16. Thirty-six of 63 patients had abnormal MRI. Ten patients had atrophy, followed by 5 multiple, small lesion in white matter, 4 space occupying leisons. 4 developmental abnormalities, 3 hypoxic encephalopaty, 2 mesial temporal sclerosis, 2 venturicular asymmetry, 2 calcified lesion. 1 hydrocephaly, 1 vascular lesion, 1 multipl hamartomatous lesions in white matter in accordance with neurofibromatosis. One patient with hyperintense symmetrical lesions in occipital lobe could not he classified.
CONCLUSION: We claim that MRI is a very important tool revealing the etiology in patients with partial seziures MRI allows us to determine the migration ubnormalities and demyelinating lesions which could not be shown in CT.

Parsiyel Epilepsilerde Magnetik Rezonans Bulguları

AMAÇ: Bu çalışmada parsiyel epilepsi tanısı oları 63 hastada Magnetik Rezonans (MR) görüntülemenin etyolojiyi belirlemedeki yeri araştırılmıştır.
MATERYEL VE METOD: Şişli Etfal Hastanesi Nöroloji Kliniği Polikliniğinde parsiyel epilepsi tanısı ile izlenen 63 hastanın MR bulguları retrospektif olarak değerlendirildi. Hastalardaki nöbet tipleri hem kendilerinden alınan anamnez, hem de nöbeti yakından izlemiş olan kişilerden edinilen bilgiler ışığında belirlendi. Hastaların tümünde elektroansefalografi (EEG) yılda en az iki kez yinelendi. MR incelemesinde T1, T2 ve proton yoğunluğunda aksiyal,koronal ve sagital kesitler alındı.
BULGULAR: Hastaların 32'si kadın, 31 tanesi erkek olarak belirlendi. Nöbet tipi 6 hastada basit parsiyel, 14 hastada kompleks parsiyel ve 43 hastada sekonder jeneralize olarak saptandı. Elektroamefalografik (EEG) incelemelerde 47 hastada patoloji bulundu. 16 hastada ise EEG nor­maldi. MR 36 hastada patoloji gösterdi. MR bulguları 10 hastada arroji, 5 lıastada mu/tipi ak madde lezyonu, 4 has- tada yer kaplayıcı le: ymı, 4 hastada gelişimsel anomali, 3 hastada lıipoksik aıısejcılopati, 2 hastada mezial temporal skleroz, 2 hastada ventrikül asimetrisi, 2 hastada eski kalsifiye lezyon, 1 hastada hidrosefali, 1 hastada vasküler lezyon, 1 hastada nörofibromatozis ile uyumlu multipl nodüler ak madde lezyonu olarak sınıflandırıldı. Her iki oksipital bölgede az çok simetrik hiperintens iki lezyonu olan hastaya ise kesin tanı konulamadı.
SONUÇ: Parsiyel epilepsi tanısı alanlarda görüntülemede ilk seçilecek yöntem MR olmalıdır. MR bulguları, kraniyal bilgisayarlı tomografide (BT) gösterilemeyen migrasyon anmııa/ileri, mesial temporal skleroz ve demyelinizasyonla giden proçeslerin tanınmasına olanak sağlar.
