Comparison of hemodynamic parameters and recovery characteristics of propofol and etomidate in tiva

OBJECTIVE: In this study, we aimed to compare the effects of propofol and etomidate TIVA on hemoynamic parameters and recovery time.
STUDY DESIGN: Thirty patients of ASA I, II were divided into two groups. In Group I, 2 mgr/kg fentanyIIV was used before induction and anesthesia was induced with 2- 2.5 mg/kg propofol and 0.1 mg/kg vecuronium. During maintenance of anesthesia propofol was infused at a rate of 10 mg/kg/h during first 10 minutes, 8 mg/kg/h during second 10 minutes and 6mglkglh thereafter. In addition 2 mgr/kg/h fentanyl was infused in %3002% 70 air mixture. Vecuronium was used when needed. Group II received the same anesthesia with etomidate 0.3 mg/kg at induction and 20 *gr/kg/min at maintenance. Before and after induction, after intubation, just before surgical intervention, preoperatively (at every 5 minutes) and after extubation systolic, diastolic, mean arterial blood pressure and heart rate of all patients were noted. In the postoperative period the patients were observed for the recovery and wake up scores.
RESULTS: There were significant decrease and increase in hemodynamic parameters in Group I when compared with other groups (p<0.05). Time to recovery was longer in Group 11 than the other groups. (p<0.05).
CONCLUSION: Finally, TIVA with etomidate is more stable hemodynamically than propofol, but recovery time is shorter in propofol TIVA.

Total İntravenöz Anestezide Propofol ve Etomidat’ın HemodinamikParametreler ve Derlenme Kriterleri Üzerine Etkilerinin Karşılaştırılması

AMAÇ: Bu çalışmada TlVA‘da propofol ve etomidatı kardiyovasküler stabilité ve derlenme açısından karşılaştırmayı amaçladık.
MATERYAL VE METOD: ASA I, II sınıfına dahil 30 hasta 2 gruba ayrıldı. Grup I‘de indüksiyondan önce 2 mgr/kg fentanil İV verildi ve 2-2,5 Mgr/kg propofol ve 0.1 mg/kg vekuronyum ile indüksiyon gerçekleştirildi. Anestezi idamesi ilk 10 dakikada 10 mg/kglsaat; ikinci 10 dakikada 8 mg/kg/saat, cerrahi boyunca 6 mg/kg/saat propofol infüzyonu ve 2 * gri kgl saat fentanil infüzyonu ile sürdürüldü. Yine idamede %30 O2%70 hava ve gereğinde bolus vekuronyum verildi. Grup H‘ye Grup 1 ile aynı anestezi indüksiyonda 0.3 mg/kg idamesi-ise 20 *gr/kgldk etomidat ile uygulandı. Tüm hastaların SAB, DAB, OAB ve KAH değerleri; indüksiyon öncesi sonrası, entübasyon sonrası, cerrahi başlangıç, intraoperatif her 5 dakikada bir ve ekstübasyon sonrası kaydedildi. Derlenme ve uyanıklıkskorları ölçüldü.
BULGULAR: Grup I‘de hemodinamik parametrelerde istatistiksel olarak anlamlı değişiklikler daha sık görüldü ( p<0.05). Derlenme süreleri ise Grup l‘de anlamlı olarak daha kısa bulundu (p<0.05).
SONUÇ: Sonuç olarak, etomidatın hemodinamik dengeyi propofole göre daha iyi koruduğu derlenme ve uyanma süresi olarak ise propofolün etomidata üstün olduğu görüldü.
